You will have access to most of your health care services at a multi-purpose center that provides health care, activities, nutritious meals, and the opportunity to socialize and make new friends.
Support is available by phone 24 hours a day, 7 days a week by calling us at (239) 985-6400. Our regular Care Center hours are Monday through Friday from 8:00 a.m. until 5:00 p.m.
Care Centers are closed on New Year’s Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day and Christmas Day. For other PACE providers in Florida, please visit www.flpace.org. You will be leaving the Hope PACE website to view this page.

No, most program participants remain safely in their own homes. You may visit our Hope Care Center during the day.
Most of our participants come to our center two or three times a week. Others may come for a full week. Some may only attend a few days a month. Our Care Team will help create a plan with you to determine how often you will visit the Hope Care Center.
Absolutely. We have bilingual staff members who can assist our Spanish-speaking participants.
We make sure that you have the necessary care you need 24 hours a day, 7 days a week, 365 days a year. You will never be without access to assistance.
No. If you join Hope PACE, you will receive your Part D-covered drugs and all other necessary medication through our program. You will not need to join a separate Medicare drug plan.
If you need nursing home services, Hope PACE will assist you in exploring appropriate alternatives for your living arrangements in the community. You will continue to receive our care.
You may leave Hope PACE at any time. Enrollment and disenrollment occurs at the beginning of each month.
