To be eligible to enroll in Hope PACE you must:
- Be at least 55 years old
- Be a resident of Charlotte, Collier or Lee county
- Be able to live in the community without jeopardizing your health or safety
- Have health problems placing you at risk for nursing home care
The program is free of charge for participants with Medicaid coverage. For participants who are not Medicaid eligible, there is a fee. The program is also available to private pay participants. Ask us for an estimate of your costs.
Apply for Medicaid in Florida – in order to qualify, you must be a Florida resident. You will be leaving the Hope PACE website to view this page.
Participants cannot be out of our three-county service area for more than 30 days.
How Long Does it Take to Enroll?
The time it takes to enroll in Hope PACE will vary due to personal circumstances. Most participants will complete enrollment in 4 – 12 weeks, depending on Medicaid eligibility and the wait time for the required assessment. If an enrollee would like to appoint a person to file a grievance, request a coverage determination, or request an appeal on his or her behalf, the enrollee and the person accepting the appointment must fill out this form (or a written equivalent) and submit it with the request.
