Our Approach

We’re different from any other program or service.
We focus on care at home instead of going to a nursing home or other care facility.

When you enroll in Hope PACE, we handle everything you need—your primary and specialty medical care, medications, homecare, socialization and so much more, all designed to help keep you living safely and independently in your home.

Avoiding Unnecessary Hospitalizations

Aging adults are more likely to get an infection than the general public. Hospital-acquired infections are especially dangerous. When seniors are hospitalized, they lose strength quickly. They may never recover completely.

Hope PACE Helps You Stay Safely at Home
To minimize the risk of a serious infection, PACE is designed to keep you away from emergency rooms and hospitals. As a PACE participant, you can get quick medical help with a simple phone call. When needed, you will receive direct support in your home.

Participants and families can always call Hope PACE 24 hours a day, 7 days a week.

PACE participants have the right to access emergency care without prior authorization.

The mission of Hope PACE is to provide client-centered holistic long term care to frail elderly individuals while supporting their core desires for dignity, autonomy, and the ability to maintain a degree of independence.
